Cartilage lives in an enclosed microenvironment, so once injured, a vicious cycle sets in that accelerates degeneration and makes self-repair extremely difficult. We developed NEST — a biomimetic hydrogel that breaks this cycle and promotes endogenous regeneration. NEST is composed of a hydrogel wrapped with four fusion proteins produced by engineered bacteria, exploiting anisotropic properties that mimic the native cartilage structure. The specialized linkages between functional proteins and the SELP backbone, together with the cargo module design, turn NEST into a multidimensional programmable platform. It shows broad application prospects in treating osteoarthritis, artificial ligament repair, microfracture, sports injuries and more. We have also designed dedicated surgical tools to improve NEST’s operability and patient compliance.
