Title: ProteaSign, Protease Activity as a Chronic Wound Diagnostic Description: A chronic wound is assessed today much as it was fifty years ago. That is, by eye, with a ruler, and by waiting. That tells a clinician what a wound looks like, but ignores what it is doing. By the time the appearance changes, the opportunity to intervene has often passed and opportunities for treatment are wasted. In wounds that fail to heal, two protease families are particularly expressed: matrix metalloproteinases and neutrophil elastase. They are the mechanism by which healing stalls. In a healing wound, there is a balance between these proteases in their active and inactive state as they clean damaged tissue and promote cell movement and growth; in a non-healing wound, their activity is elevated and they destroy the extracellular scaffold and growth factors, exacerbating damage. However, no routine test measures them. ProteaSign is a multiplexed electrochemical diagnostic that does. Peptide substrates carrying a methylene blue reporter are immobilised on gold electrodes; when a target protease cleaves them the signal falls, and the drop is read by square-wave voltammetry in a low-cost handheld reader. A Bayesian model deconvolutes the mixed signal back to the activity of each enzyme. This video covers the clinical problem, the substrate design and specificity work, the electrochemistry, the hardware, and the regulatory and manufacturing path a device like this has to walk. It also covers what we got wrong, and how talking to clinicians, patients and regulators changed the project. Wiki | Official Page
