<description>Title: Re-CDI Alert - A Living Alert System for CDI Recurrence Description: Clostridioides difficile infection (CDI) presents a major clinical challenge due to high recurrence rates and delayed post-treatment diagnostics. Current management relies on post-symptomatic reactive care, which further disrupts gut microbiota via broad-spectrum antibiotics. To address this, team China_Guangdong_Shimen developed "Re-CDI Alert," a living biotherapeutic alert system for proactive CDI recurrence monitoring. We engineered probiotic Escherichia coli Nissle 1917 (EcN) incorporating the heterologous PcrSR-Pcmh two-component sensing system from Aromatoleum aromaticum EbN1. This engineered strain selectively detects p-cresol—a characteristic metabolite produced during C. difficile germination—and converts local chemical signals into quantifiable reporter outputs (sfGFP). Furthermore, the system serves as a modular chassis for on-demand therapeutic release, such as neutralizing nanobodies. Re-CDI Alert shifts the paradigm from delayed reactive treatment to continuous proactive risk sensing, providing a safer, precise diagnostic platform to safeguard gut microecosystem health.
